Privacy of Personal information is an important
principle to Therapy Health Care Inc. We
are committed to collecting, using and disclosing personal information
responsibly and only to the extent necessary for the services we provide. We also try to be open and transparent as to
how we handle personal information.
Therapy Health Care and its personnel are committed to collecting, using
and disclosing personal information about you responsibly and only to the
extent necessary to provide you with the health services that you require.
Personal information about you may be shared within the organization,
but on a limited and need to know basis only (e.g. with another therapist from
our agency directly involved in your care, with a therapy leader or manager
occasionally). Also, our office
personnel have limited and supervised access to certain types of information
but only as is required in order to
deliver services to you (e.g. sorting and faxing referrals to therapists,
filing, and inputting billing data).
Our organization (and occasionally external consultants hired by us),
reviews client and other files periodically, for the purpose of ensuring that
we provide high quality services and to assess the performance of our
therapists and staff.
If the cost of your services is being paid by a 3rd party
(e.g. CCAC, WSIB, Motor Vehicle Insurance), then these payers often have your
consent or legal authority to direct us to collect and share certain
information about you with them.
There
are certain circumstances where we may be required to share your personal
information. These include: If requested by our therapists’ Professional
Colleges as part of their regulatory activities; if requested by various government agencies, such
as Human Rights Commission, Information and Privacy Commissioner, in the event
that serious illegal behaviour is reported; in the
event that our business is sold (in which case the purchaser would have to
provide a written promise that the information be kept confidential).
Your personal information is kept confidential at all times. It is stored securely and only for as long as
is required by our therapists’ Professional Colleges, after which time it is
destroyed.
You have the right to look at your information and to ask for
corrections to any factual information, if you believe that there is a mistake.
